Sample Demographic Data Collection Tools for Use in BFNEP Programs

Here are four different data collections tools that were developed to help you begin collecting demographic information from BFNEP participants. You may use these tools separately, or you can cut-and-paste into your pre-existing surveys. You may also reformat the tools (e.g., increase font size) to meet your individual needs.
 

1—Observation
This tool is a tally sheet that can be used by nutrition instructors or volunteers to collect information by observing a program. When filling out the tool you may need to provide your best guess or estimate of the total number of people in each category.

2—Individual Format A Landscape
This tool is to be filled-out by individual participants. It can be copied and given to participants separately to fill-out or added to the END of pre-existing questionnaires/surveys.

3—Individual Format B Portrait
This tool is to be filled-out by individual participants. It can be copied and given to participants separately to fill-out or added to the END of pre-existing questionnaires/surveys.

4—Multiple Participants
This tool should be given to individuals who are providing information for group of people or family members (for example—participants at commodity distribution centers, food bank patrons, Head Start, ECEAP, or WIC clients). It can be given to participants as a separate questionnaire or added to the END of pre-existing questionnaires/surveys. You may replace the phrase “family members” with “people” or some other word if it is more appropriate.
