Step 1:  Meet the challenge eligibility requirements

Step 2:  Complete all the requirements provided in the information including necessary education provided in the Washington State Specific Objectives appropriate for your certification level and Infectious Disease Prevention training.

Step 3:  Complete the Challenge Certification Application
This form may be filled out on your screen, however, it MUST be printed out to obtain the necessary signatures.

Step 4:  Provide the information to this office


EMT-Basic

Step 1:  Meet the challenge eligibility requirements
