Step 1:  Complete all Continuing Medical Education (CME) or Ongoing Training and Education Program (OTEP) requirements

Step 2:  Complete the Recertification Application
This form may be filled out on your screen, however, it MUST be printed out to obtain the necessary signatures.

Step 3:  Provide the information to this office

EMT-Basic

Step 1:  Complete all Continuing Medical Education (CME) or Ongoing Training and Education Program (OTEP) requirements

Step 2:  Complete the Recertification Application

Intravenous Therapy Technician
